Low Income Subsidy (LIS) Information:
Low Income Subsidy (LIS) also knows as Extra Help is a program run by the Social Security Administration that helps reduce the cost of a consumer's plan and prescription drugs.
There are 4 levels of LIS which a consumer can qualify for based on their income. There is a percentage associated with each level of LIS and the percentage determines how much of the plan premium LIS will help pay for and the amount of money the consumer will pay for their prescriptions/copays.
Levels 1 - 3 = 100%
Level 4 = 25%, 50%, 75%, or 100% (varies based on the consumer eligibility)

Cost Sharing Plan Information:
When a consumer has partial or inactive Medicaid eligibility you must inform the prospective member of the potential co-pay/co-insurance amounts they could incur if they enroll in a cost-sharing plan without having a level of Medicaid that would help cover plan costs.

Chronic Condition Eligibility
In order to enroll a consumer in a UnitedHealthCare Medicare Silver or UnitedHealthCare Chronic Complete plan the consumer must have Medicare, Medicaid AND a qualifying chronic health condition such as a cardiovascular disorder and/or chronic heart failure.
Please confirm the consumer meets these requirements before enrolling them in one of these plans.
