Great news! The ability to look up clients on the Medicare & Medicaid Eligibility Lookup tool has been restored for many states for the 2025 plan year, with more coming soon. To verify eligibility in states that are not yet available, please reach out to the Producer Help Desk (PHD) at 888-381-8581. Your lookup request will be processed within one business day. 

 

The PHD is open Monday-Friday 7:00 a.m. - 9:00 p.m. CT. After hours, please submit your request via Jarvis PHD Chat, which is available 24/7.

While our Producer Help Desk (PHD) continues to be available to help confirm eligibility and assist in understanding what plans a consumer is eligible for, you can now use the tool to verify Medicaid eligibility for some states before submitting an application any time, any day without having to call PHD. 

Helpful reminders:

• All D-SNP enrollments submitted previously will go through an eligibility check, so there is no need to resubmit any applications that are processing. 
• Medicaid verification is not required to submit D-SNP applications to UnitedHealthcare. However, a Medicaid ID is required for our enrollment teams to verify a consumer’s current Medicaid eligibility and helps ensure a faster processing time.

Steps to verify eligibility

1. From the Jarvis homepage, select Medicare & Medicaid Eligibility Lookup
   
   
   
   
2. Within the page, choose Medicare Eligibility or Medicaid Eligibility  
   • Fill out the required information  
   • Click Check Medicaid Eligibility to get the results
     • This button will work once all the information is entered
